Medicaid Waiver Application Completion Session Facilitator – Janet Smola

ExOPS has hosted many sessions concerning the what and the why of the importance of applying for a Medicaid waiver in order to access government funding for your loved one with a developmental disability. The subject is a complicated one but the application process is relatively easy; though can take months to be approved. Medicaid waivers should be applied for long before funding is actually needed. There is a long waiting list for funding for services and often times a person can be on the waiting list for years. Currently funding is being distributed by a “most in need” criteria.

We have assembled a panel of volunteers who are familiar with the application process to be present for this meeting to actually sit down and help you fill out the forms. We will be able to make copies for you, provide mailing supplies and will give you step by step instructions on how to fill out the forms. Our goal is for you to leave this meeting with the completed application in hand. You can access the actual application by visiting our web site. For those of you who do not have access to the internet, or a printer, I’ll make a copy for you here. To expedite the process, bring the following required documents (for the person for whom you are applying) with you:

• Proof of Citizenship – birth certificate, passport, permanent resident card • Copy of Social Security Card or Social Security Number • Copy of Medicaid and/or Medicare card • Copy of Social Security Benefit information • Current doctor’s names, addresses, and telephone numbers

This meeting will be especially important for parents of children with disabilities who are high school aged and older. Our meeting will begin at 7:00pm. Refreshments will be available and children are welcome in the care of their parents. We’ll meet in our offices off of Highway 54 near Publix in Peachtree City.
